pci id subsystem id
1002:6600 103c 1952 - mobile discrete gpu: Radeon HD 8k
1002:9902 103c 1952 - mobile integrated APU Radeon HD 7k gpu when mobile discrete gpu is enabled
1002:9902 103c 194e - mobile integrated APU Radeon HD 7k gpu when mobile discrete gpu is disabled
So when discrete gpu is enabled 103c 1952 subsystem id is shared between mGPU i dGPU. When dGPU is disabled mGPU have mainboard subsystem id: 103c 194e.
194e is mainboard/laptop subsystem id
1952 is shared between both Radeon HD only. No other component on this laptop uses this id.

Radeon HD 8k or 7k is not a full graphics card model. Besides, the 1002:9903 ID already makes clear that it's a Radeon HD 7640G so at least the integrated graphics does not need a Radeon HD model in its subsystem (unless the 7640G name is incorrect).
Please don't put your own name into the Name field.

@ xVekx: both cards already have a ProBook 455 G1 subsystem already.
In case you wanted to post data about your discrete card (for example its model number 8670 or 8750), please go to the 1002:6600:103c 1952 entry instead.
